EFT Payment Authorization

Client hereby authorizes Sam Miller and/or Oracle Athletic Science LLC, to make periodic charges or withdrawals (EFT Authorization) from the account or card provided and designated by the client, and accepted by Sam Miller, Oracle Athletic Science LLC, for the payment of any and all fees, late fees, costs, expenses incurred from client's chosen program.
